Department: 34319 Carlyle Place Navicent Health - Skilled Nursing Facility Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information: 7p- 7am Pay Range: $25.30 - $37.95 Licensed Practical Nurse (LPN) - Skilled Nursing & Senior Living Carlyle Place 5300 Zebulon Rd, Macon GA Schedule Night Shift Three (3) 12-hour shifts (7:00 PM to 7:00 AM) One (1) 6-hour shift each week Every other weekend rotation Location Carlyle Place, part of Atrium Health Navicent, is a premier not-for-profit Life Plan Community located in northwest Macon, Georgia. Position Summary The Licensed Practical Nurse (LPN) provides nursing care and assists in the planning and implementation of resident care focused on comfort, wellness, and quality outcomes. Working under the supervision of a Registered Nurse or licensed practitioner, the LPN participates in assessments, delivers treatments, monitors resident conditions, and contributes to individualized care plans. Key Responsibilities Provide compassionate nursing care to residents in Skilled Nursing, Assisted Living, and Memory Care settings. Monitor resident conditions and report significant changes to the healthcare team. Assist with assessments, care planning, treatments, and procedures. Administer medications and treatments within scope of practice. Educate residents and families regarding care plans and treatments. Document assessments, interventions, and resident responses accurately and timely. Collaborate with interdisciplinary team members to support positive resident outcomes. Ensure care is delivered safely, efficiently, and in accordance with regulatory requirements. Qualifications Graduate of an accredited School of Practical Nursing. Current, unrestricted Georgia LPN license (or compact license eligible for practice in Georgia). Current CPR certification or ability to obtain within 60 days of hire. Basic Life Support (BLS) certification required per policy. Skilled nursing, long-term care, assisted living, or memory care experience preferred.
