Home Health Registered Nurse

Job Summary Join our dynamic healthcare team as a Home Health Registered Nurse, where you will play a vital role in delivering compassionate, expert nursing care directly in patients’ homes. Responsibilities • Conduct comprehensive patient assessments including vital signs, physical examinations, and detailed health histories to determine individualized care needs. • Administer medications via injections, IV infusions, and other routes following aseptic techniques and safety protocols. • Manage complex wound care procedures such as suturing, ostomy care, and dressing changes while maintaining strict infection control standards. • Utilize electronic health record (EHR) systems like Epic or Cerner to document patient encounters accurately and efficiently. • Provide specialized care for patients with chronic conditions including cardiac catheterization recovery, dialysis management, and long-term care needs. • Educate patients and their families on disease management, medication administration, nutrition, and safety measures to promote self-care. • Collaborate with physicians and interdisciplinary teams for diagnostic evaluations, discharge planning, and case management to ensure seamless continuity of care. Qualifications • Valid RN license with current registration in the state of practice. • Demonstrated experience working with individuals with disabilities, memory care needs, or behavioral health challenges. • Proficiency in EMR/EHR systems such as Epic or Cerner; familiarity with ICD-10/ICD-9 coding is preferred. • Critical care experience including ICU or trauma center background is highly desirable. • Knowledge of advanced nursing procedures such as IV insertion, ventilator management, tube feeding, and aseptic technique. • Strong assessment skills with the ability to perform physical examinations and interpret laboratory results. • Experience working in diverse settings including hospital medicine, emergency medicine, hospice & palliative medicine, or skilled nursing facilities such as nursing homes or assisted living communities.
