Overview

We are seeking a skilled and compassionate Registered Nurse to join our home health team. The ideal candidate will have a strong background in patient care and a passion for helping individuals maintain their independence at home.

Key

Responsibilities

Conduct comprehensive assessments of patients in their homes. Develop and implement personalized care plans. Administer medications and treatments as prescribed. Monitor patient progress and make adjustments to care plans as needed. Educate patients and families on health management and disease prevention. Collaborate with a multidisciplinary team to ensure holistic care. Maintain accurate and timely documentation of patient care.

Qualifications

Current RN license in Massachusetts. Home health experience preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively. Valid driver’s license and reliable transportation.
