We are seeking a compassionate and experienced Hospice Registered Nurse Case Manager (RN) to join our team at Ascend Hospice in Topeka, KS. About the Role
As a Hospice Registered Nurse Case Manager (RN) , you will be responsible for assisting the team with on-call, case management, and visits to ensure adequate coverage and call-outs. Your duties will include but are not limited to:
- Completing initial assessments of patients and families to determine hospice care needs
- Regularly re-evaluating patients' nursing needs
- Using assessment data to determine care plan needs
- Collaborating with the healthcare team, patient, and family to direct the work of the team to provide continuous and well-coordinated care
- Counseling patients and families in meeting end-of-life needs
- Initiating appropriate preventative and palliative nursing procedures
- Communicating with physicians regarding patients' needs and reporting any changes in patients' conditions
- Supervising Hospice Aides

Requirements
- Associate's Degree in Nursing
- Must have a State Certified RN license
- 2 years' experience as a professional nurse, hospice experience preferred
- Must have a valid Driver's license and reliable transportation
- Must be willing to travel
- Strong verbal and written communication skills
