About this role

Overview

The RN Clinic Care Coordinator directs, coordinates, and monitors clinical activities within an ambulatory clinic setting. This individual will interface with the multidisciplinary health care team to ensure care coordination and delivery of needed care services. This individual closely collaborates with department physicians and clinic leadership to ensure care coordination actions are consistent with the goals of patient care and the practice. Assist patients and caregivers in navigating appointment referrals and follow up issues.
