Palm Beach Nursing Center is seeking a skilled and detail-oriented MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process. The ideal candidate will be well-versed in PDPM and Medicare documentation, ensuring compliance with state and federal regulations while supporting exceptional resident care. Key responsibilities include completing and coordinating MDS assessments, developing individualized care plans, collaborating with interdisciplinary teams, monitoring PDPM requirements, reviewing QI/QM reports, participating in surveys and audits, submitting assessments electronically, and educating staff on MDS/PDPM updates.

Requirements

Active RN license in the state of Florida
Active RN license in the state of Florida
Proven experience with PDPM and Medicare documentation
Minimum of 2 years' experience in a skilled nursing facility preferred
Strong knowledge of MDS 3.0, care planning, and RAI processes
Excellent communication, leadership, and organizational skills
