About this role

At Granite VNA, hospice nursing is about more than providing clinical care—it's about bringing comfort, dignity, and compassion to patients and their families during one of life's most meaningful journeys. As a Hospice RN, you'll be a trusted guide, advocate, and clinical expert, working alongside an interdisciplinary team dedicated to exceptional end-of-life care. Build meaningful relationships with patients and families while explaining the hospice philosophy, available services, program expectations, and bereavement support. Complete comprehensive patient assessments and develop individualized Plans of Care in collaboration with physicians, patients, caregivers, and the interdisciplinary team. Deliver skilled nursing care that promotes comfort, symptom management, and quality of life. Provide clinical supervision and support to LPNs and LNAs. Coach, mentor, and support new nursing team members and nursing students.

Requirements

  • Graduate of an accredited school of nursing
  • Current New Hampshire nursing license or compact state license through Nurse Licensure Compact (NLC)
  • Three to four years of recent general medical/surgical or skilled/acute care nursing experience
  • RN License
  • End-of-life care experience
  • Home health experience
  • Acute care experience
  • Medical-surgical experience
