The Community Nurse Navigator will provide care coordination, follow up and support to patients in an effort to improve their breast and cervical health outcomes. This position will lead the program activities with the navigator working in the Equal Hope office. Responsibilities

- Patient Navigation Works with the project director to develop protocols to receive abnormal mammography results from each participating site.
- Reaches out to women with abnormal mammograms to assist them with follow-up.
- Offers guidance and emotional support to patients regarding decisions on follow-up and help to complete diagnostic process for breast and cervical cancer.
- Arranges appointments with patients, reviews test preparation procedures and assesses potential patient barriers to ensure the patient makes it to the diagnostic mammography and, as indicated, completes the breast cancer diagnostic process.
- Contacts patients via phone calls, emails and letters to remind them about appointments, assist in scheduling/rescheduling appointments and to answer questions about breast cancer diagnostic imaging, biopsies and treatment.
- Performs follow-up phone calls to patients after appointments as indicated.
