Beyond Hospice , a service of Episcopal Homes of Minnesota, is seeking an experienced and compassionate Registered Nurse Case Manager to provide end-of-life care and support to patients and their families.
This is a great opportunity for an RN who enjoys working independently while being part of a supportive interdisciplinary team.
Pay & Schedule
Pay: $37.44–$56.66 per hour, based on experience
Schedule
Monday–Friday, 8:00 a.m.–4:30 p.m.
Weekend Rotation: Every 6th weekend
What You’ll Do
Complete initial and ongoing nursing assessments and develop individualized plans of care.
Provide skilled nursing care focused on comfort, symptom management, and quality of life.
Administer medications and treatments and complete medication reconciliation.
Serve as Case Manager for assigned patients, coordinating all aspects of hospice care.
Educate and support patients, families, and caregivers throughout the hospice journey.
Coordinate medications, medical equipment, supplies, and community resources.
Collaborate with physicians, social workers, chaplains, therapists, and other members of the interdisciplinary team.

Required

Two years of acute care nursing experience.
Two years of recent hospice or home health experience.
Physical Requirements
