About the Position
As a Home Health RN, you will provide skilled nursing care to patients in their homes. You’ll work independently while collaborating with patients, families, physicians, and our care team to ensure each patient receives quality, individualized care.

Responsibilities
- Provide skilled nursing care in patients’ homes
- Complete comprehensive nursing assessments
- Monitor patients’ health status and identify changes in condition
- Administer medications and treatments as ordered
- Develop and follow individualized plans of care
- Communicate with physicians, families, and other members of the care team
- Provide patient and family education
- Complete accurate and timely clinical documentation
- Coordinate care and communicate changes in patient condition
- Promote patient safety, independence, and quality of life
What We’re Looking For
- Current RN license in the State of Ohio
- Home health experience preferred , but we are willing to consider nurses with strong clinical experience in other settings
- Strong assessment and clinical skills
- Excellent communication and organizational skills
- Ability to work independently and manage your schedule effectively
- Reliable transportation
- Compassionate, dependable, and patient-centered approach to care
