Job Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our home health team on a per diem basis. In this vital role, you will provide high-quality, personalized nursing care directly in patients’ homes, promoting health, healing, and independence. Your expertise will ensure that each patient receives comprehensive assessments, tailored treatment plans, and compassionate support to improve their quality of life. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations across various settings.
Duties
• Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to develop individualized care plans.
• Administer medications and injections accurately, including IV infusions and medication management aligned with best practices and patient needs.
• Provide specialized care such as wound suturing, ostomy care, catheterization, and ventilator management while maintaining strict aseptic techniques.
• Coordinate with interdisciplinary teams to ensure seamless discharge planning, case management, and adherence to healthcare standards such as NCQA and HIPAA regulations.
• Educate patients and their families on disease management, medication administration, nutrition, and safety measures to promote health literacy.
• Utilize electronic health record (EHR) systems like Epic or Cerner for accurate documentation of patient care activities and medical records.
• Respond promptly to urgent situations through triage protocols, acute pain management procedures, and emergency response techniques including basic life support (BLS).
Requirements
• Valid RN license in the state of practice with current CPR certification.
• Proven experience working in home health care or related settings such as nursing homes, assisted living facilities, or hospice environments.
• Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and diagnostic evaluation processes.
• Hands-on experience with EMR/EHR systems such as Epic or Cerner for efficient documentation and patient tracking.
• Competency in performing complex procedures including IV insertion, phlebotomy, laboratory specimen collection/processing, and sterile processing techniques.
• Critical care experience preferred, especially within ICU or trauma center environments; familiarity with trauma medicine and infectious disease care is advantageous.
• Ability to work independently while managing multiple patient cases with excellent organizational skills.
• Knowledge of Workers' Compensation law and experience working with individuals with disabilities or developmental disabilities is a plus.
