About the Role
As a Wound Care Coordinator, you will utilize the nursing process for wound care, conduct skin integrity rounds, and develop and implement wound care prevention plans. You will also document residents' skin condition, communicate with physicians, and initiate nutritional interventions to promote wound healing. In addition, you will perform direct resident care, administer treatments, and ensure proper care is provided safely in accordance with facility policies and procedures.

Responsibilities
- Utilize the nursing process for wound care, including assessment, planning, implementation, and evaluation
- Conduct skin integrity rounds, wound measurements, care plan reviews, and treatment recommendations
- Document residents' skin condition and medical history related to wound care
- Develop and implement wound care prevention plans
- Set priorities for wound care nursing actions to ensure effective treatment
- Communicate with physicians regarding wound care needs and necessary interventions
- Initiate nutritional interventions to promote wound healing
- Perform direct resident care, following established procedures and policies
- Administer treatments and procedures as per physician's orders
- Conduct rounds to ensure proper pericare and application of preventative ointments for incontinent residents
- Ensure all duties are performed safely in accordance with facility policies and procedures
Requirements
- Active Nursing License as required by the state
- Current CPR certification
- Minimum 2 years of experience as an LPN/RN
- Ability to lift and move patients/items weighing at least 75 pounds
- Strong vision abilities including close vision, distance vision, color vision, peripheral vision, and depth perception
- Must be able to speak, read, and write in English
- Ability to cope with mental and emotional stress in a fast-paced healthcare environment
Salary: $42 - $48 HOURLY
