Chelsea's Home Health Care Inc. is hiring LVN Case Manager to provide excellent care to pediatric or Adult patients who are dependent on G-tube, Tracheostomy, and/or Ventilator.

Job Overview

LVN Case Manager who has Experience in Home Health Private Duty Nursing for patients that are dependent on G-tube, Tracheostomy and Ventilator. LVN must be experienced with Pediatrics and Adult patients. We may begin by sending you to do SOC or do a new intake assessment in all areas of Los Angeles and the surrounding cities.

At first you may begin with 5 to 10 patients Case Management. Telehealth is provided as well as going to patient's home for recertification and assessment. You may be required to teach LVNs on complex procedures such G-tube, oxygen treatment, Tracheostomy suctioning and Ventilator circuit monitoring.

We are willing to provide training shadowing as well as conducting comprehensive plan of care. customer service skills/compassionate explaining to families of patient or to patient the progress of care and the purpose of home health and LVNs working in the home. You can work from home or in the office or mix. We are willing to Negotiate.

Instructing field LVNs the scope practice of LVNs within a home.

Training: We provide paid training for all new LVN Case Manager nurses to be proficient in G-tube, Tracheostomy and Ventilator.

Requierments

  • Current LVN/LPN License
  • Valid CPR Card
  • Current Physical Exam (Valid up to 1 year)
  • Current TB Test Results (Valid up to 1 year) OR Current Chest X-Ray (Valid for up to 2 yrs)
  • Job Type: Full-time
  • Pay: $30.00 - $35.00 per hour
  • Work Location: In person
