Role: Registered Nurse in a long-term care and rehabilitation community.
Responsibilities
Perform comprehensive assessments of residents
Administer medications and treatments
Coordinate individualized care plans with an interdisciplinary team
Monitor resident conditions and document accurately in EHR
Support rehabilitation and chronic care objectives
Guide and support support staff
Qualifications & Skills
Valid RN licensure
Strong assessment and clinical skills
Excellent communication and teamwork abilities
Ability to provide compassionate, resident-centered care
Knowledge of geriatric and rehabilitative nursing practices
