Responsibilities
Provide direct patient care, including administering medications, performing vital signs, and assisting with daily living activities
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor and report patient progress, and make recommendations for changes in care
Collaborate with other healthcare professionals to ensure seamless care transitions
Maintain accurate and detailed records of patient care and progress
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Provide emotional support and comfort to patients and families
