What if you could keep doing the nursing you love—without living on the hospital floor?

You’re an experienced RN. You know how to assess a situation, recognize when something isn't right, talk with a worried family member, coach a caregiver through a difficult situation, and figure out what needs to happen next.

But maybe you're ready for something different.

A role where you can use your clinical judgment without spending your entire day managing a hospital assignment. A role where you actually get to know your clients and families. A role where every day is a little different, you're trusted to work independently, and the work you do makes a visible difference.

That's what this role is about.

SYNERGY HomeCare Northeast Metro is looking for an experienced RN to join our team as a Client Care Coordinator.

This is a staff-level RN position—not a DON or nursing management role. You'll work closely with our Director of Nursing and the rest of our team, bringing your nursing experience and judgment into a home-care environment where relationships, communication, and problem-solving matter just as much as clinical knowledge.

Your day won't look the same every day.

One day you may be completing a client assessment or supervisory visit. Another day you might be helping a Care Professional safely transfer a client, talking with a family about a change in condition, updating a Plan of Care, or working with our team to solve a problem before it becomes a bigger one.

You'll spend some time in the office and some time in clients' homes. You'll work with older adults, families, Care Professionals, and the office team. You'll train and support Care Professionals, participate in new-hire clinical orientation, and help make sure the people providing care have the knowledge and skills they need to do their jobs well.

You'll also have a voice in improving the way we provide care. This includes participating in our Care Professional Advisory Panel, following up on client and Care Professional feedback, and helping identify ways we can make the experience better for everyone.

We're looking for an RN who thinks beyond the task.

You don't need someone standing over your shoulder telling you what to do next. You can walk into a situation, gather the information, use your clinical judgment, determine the appropriate next step, and communicate clearly.

You also know your limits. You aren't afraid to say, "I'm not sure. Let me find out." You understand when you can handle something yourself and when the Director of Nursing needs to be involved.

You're calm when the day changes. You're comfortable having difficult conversations. You can be empathetic without avoiding the hard stuff. And you understand that working in someone's home requires both clinical expertise and respect for the client's choices, dignity, and independence.

Most importantly, you genuinely like people.

You enjoy older adults. You enjoy getting to know families. You don't look down on Care Professionals—you understand the important work they do and know how to support them while still holding appropriate standards.

You may be the person we're looking for if you have

  • An active Minnesota RN license
  • 3+ years of nursing experience (home care, home health, hospice, senior care, long-term care, rehabilitation, assisted living, hospital, or a similar setting)
  • Strong clinical judgment and confidence working independently
  • Experience working directly with patients and families
  • The ability to communicate clearly, professionally, and compassionately
  • Comfort with changing priorities and an occasional unpredictable day
  • Strong organization, documentation, and follow-through
  • A valid driver's license and reliable transportation
  • Basic comfort with technology, including email, Word, Google Workspace, spreadsheets, and electronic documentation

Experience with catheter care, G-tubes, colostomies, wound care, mechanical lifts, hospice, dementia, Parkinson's, falls, transfers, or other common senior-care needs is a plus.

This probably isn't the right role for you if...

You need every day to be predictable. You prefer to work strictly from a task list without much independent decision-making. You don't enjoy talking with families or having difficult conversations. Or you're looking for a nursing management position.

We're looking for someone who wants to use her experience , not simply clock in and complete a list of tasks.

What you'll get in return

You'll have the opportunity to build relationships with clients and families, work alongside a team that knows and supports one another, and use your nursing experience in a way that has a direct impact on the quality of care people receive in their homes.

You'll have independence, variety, and the support of an experienced Director of Nursing when you need it. And because this is home care, you'll have the opportunity to see the whole picture—not just the few minutes a patient spends receiving care.

  • Full-time | Monday–Friday, generally 8:00 AM–4:30 PM
  • $34–$38/hour, depending on experience and qualifications
  • Based in our Andover office with regular field visits
  • Occasional after-hours clinical support is part of the role

If this sounds like the kind of nursing you've been looking for... we'd like to meet you.

Tell us a little about your nursing experience and what has you looking for something different. We aren't just looking for someone who checks the RN boxes. We're looking for the right person to join our team and make a difference in the lives of our clients, families, and Care Professionals.
