Family Pillars Home Care is expanding our clinical team and seeking compassionate, reliable Registered Nurses to complete Start of Care (SOC), Resumption of Care (ROC), Re‑certifications, and Skilled Nursing Visits in the community.

Responsibilities

  • Complete Start of Care (SOC) assessments in accordance with state and agency guidelines
  • Perform Skilled Nursing Visits including wound care, medication management, patient education, and chronic disease monitoring
  • Develop and update individualized care plans
  • Communicate with physicians, case managers, and the interdisciplinary team
  • Document accurately and timely in the EMR
  • Provide high‑quality, compassionate care to patients in their homes

Requirements

  • Active Florida RN license
  • One year of nursing experience (home health preferred but not required)
  • Valid driver’s license and reliable transportation
  • CPR certification
  • Level 2 background screening (or willingness to obtain)
  • Strong clinical assessment skills
  • Ability to work independently
