Registered Nurse (RN) Case Manager position at FairAcres Manor. Monday – Friday 8:30am to 5:00pm with rotating weekend on-call requirements. Role involves coordinating delivery of services to Managed Care and Medicare residents, collaborating with social workers and clinical teams, orienting new residents/families, scheduling care conferences, assisting with discharge planning, communicating with managed care/insurance case managers, completing MDS social services components, providing community resources, and participating in on-call rotation.
