We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our dynamic healthcare team in. - Conduct thorough patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Records (EHR) systems such as Epic or Cerner.
- Administer medications and injections safely, including IV infusions and catheterizations, following strict aseptic techniques and medication protocols.
- Manage complex care needs such as Alzheimer's disease, dementia, memory care, hospice care, and acute pain management for diverse patient populations.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices.
- Provide emergency response support in urgent situations like airway management, ventilator operation, or trauma care within Level I or Level II trauma center environments.
- Assist with diagnostic evaluations such as specimen collection/processing and medical imaging coordination while ensuring compliance with infection control policies.
- Educate patients and families on health coaching topics like medication administration, nutrition, discharge planning, and caregiver communication.
- Document all patient interactions accurately in EMR/EHR systems while maintaining strict adherence to HIPAA and clinical confidentiality policies.
*Experience*
- Proven experience working with geriatric patients in skilled nursing facilities or assisted living communities is highly preferred.
- Prior experience with medical-surgical units or home & community care environments enhances your ability to deliver comprehensive patient care.
- Knowledge of infectious disease management, hospice services, trauma response protocols, and acute care procedures is desirable.
- Demonstrated ability to work effectively with individuals with disabilities or developmental challenges is a plus..
