Job Summary

We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our dynamic healthcare team in. Responsibilities

• Conduct thorough patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Records (EHR) systems such as Epic or Cerner.
• Administer medications and injections safely, including IV infusions and catheterizations, following strict aseptic techniques and medication protocols.
• Manage complex care needs such as Alzheimer's disease, dementia, memory care, hospice care, and acute pain management for diverse patient populations.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices.
• Provide emergency response support in urgent situations like airway management, ventilator operation, or trauma care within Level I or Level II trauma center environments.
• Assist with diagnostic evaluations such as specimen collection/processing and medical imaging coordination while ensuring compliance with infection control policies.
• Educate patients and families on health coaching topics like medication administration, nutrition, discharge planning, and caregiver communication.
• Document all patient interactions accurately in EMR/EHR systems while maintaining strict adherence to HIPAA and clinical confidentiality policies.

Experience

• Proven experience working with geriatric patients in skilled nursing facilities or assisted living communities is highly preferred.
• Prior experience with medical-surgical units or home & community care environments enhances your ability to deliver comprehensive patient care.
• Knowledge of infectious disease management, hospice services, trauma response protocols, and acute care procedures is desirable.
• Demonstrated ability to work effectively with individuals with disabilities or developmental challenges is a plus..
