Seeking experienced home health care RN's. Providing home health care services to meet the clinical needs of the individuals within facilities and their homes. Responsibilities include re-evaluating patient nursing needs, initiating plan of care, using health assessment data to determine nursing diagnosis, developing care plans, administering medications and treatments, counseling patient and family, providing health care instructions, identifying discharge planning needs, preparing clinical notes, communicating with physicians, participating in on-call duties, and ensuring equipment availability.
Requirements
Current RN IL license
CPR Certification
Minimum of 2 years of home health care experience
In-home healthcare experience
Home health agency experience
Driver's License with insured automobile in good working order
Excellent observation, verbal and written communication skills
