Our company is searching for experienced candidates for the position of Per Diem/PRN Registered Nurse (RN) Duties: • Responsible for coordination of Start of Care, Re-certification, Discharge, and Resumption of care • Provide direct patient care, including administering medications, performing diagnostic tests, and monitoring patient vital signs. • Collaborate with physicians and other healthcare professionals to develop and implement patient care plans. • Educate patients and their families about medical conditions, treatment options, and preventive care measures. • Assist with discharge planning and coordinate post-discharge care for patients. • Provide specialized care for patients with dementia or other cognitive impairments. Qualifications: 1. Valid Registered Nurse (RN) Required 2. Minimum 2 years experience in home care experience 3. Effective written and verbal communication 4. Experience with policy, rules, and regulations 5. Clean background check and driving record Note: The specific qualifications may vary depending on the employer's requirements.
