About this role

The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Completes initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.

Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient's status, initiates plan of care, develops care plan with palliative nursing actions, administers medications and treatments as prescribed, counsels patient and family, provides health care instructions, assists with activities of daily living, acts as Case Manager when assigned, communicates with physicians and community health persons, teaches self-care techniques, supervises ancillary personnel, participates in on-call duties and quality assessment performance improvement activities.
