About this role
Elizabeth City Health and Rehab is looking for a motivated Registered Nurse (RN) Minimum Data Set (MDS) Coordinator to join our compassionate team of caregivers. The MDS Coordinator completes assessments and care plans for residents using the Resident Assessment Instrument (RAI) process in accordance with the laws, regulation, and company standards. The MDS Coordinator reviews and coordinates Patient Driven Payment Model (PDPM) and Case Mix data for the facility to ensure appropriate reimbursement. This position is for an RN and is Full Time.

Requirements
- Current license as a Registered Nurse issued by the State Board of Nursing
- Experience with the RAI process
- Strong Knowledge of PDPM and State Case Mix Time weighted Reimbursement payment system.
- Knowledgeable of the nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
- Effective verbal and written English communication skills.
- Customer service oriented with the ability to work well under pressure.
- Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
- Function independently, and have flexibility, personal integrity, and the ability to work effectively with employees and vendors.
- Demonstrate basic to intermediate skills in Microsoft Word, Excel, Power Point, and Internet navigation, preferred.
- High level of professionalism with the ability to maintain confidentiality.
Responsibilities
- Coordinates the completion of the MDS by all disciplines.
- Maintains and follows a schedule of due dates for all MDSs.
- Reviews the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy.
- Completes the MDS and input into the computer.
- Verifies that assessments have been transmitted and approved by the State in a timely manner.
- Corrects any rejected records and prepares them for re-submission.
- Formulates Resident Care Plan along with interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame.
- Schedules and conducts Care Plan meetings on a regular and timely basis.
- Communicates the Care Plan to appropriate staff members and notifies supervisor if noncompliance by staff is noted.
- Works with all in-house and ancillary departments to assure understanding and compliance with the Care Plan.
- Performs other related duties as directed by the Director of Nursing Services and Administrator.
