About this role
Case Manager position at Vibra Hospital of Central Dakotas, a 41-bed Critical Care Hospital in Mandan, ND providing long-term acute care services. Responsible for coordinating clinically complex patient care across a continuum, ensuring quality clinical and cost outcomes, negotiating and coordinating appropriate services and resources, and implementing comprehensive discharge plans. $15,000 Bonus Opportunity. Clinical Career Ladder program with pay increases at Novice, Intermediate, and Advanced levels.

Requirements
- Current, valid, and active license to practice as a Registered Nurse, MSW or Licensed Social Worker in the state of employment required
- Current BLS certification from a Vibra-approved vendor required
- One (1) year of case management experience preferred
- Previous LTAC experience preferred
- CCM or ACM certification preferred
- Working knowledge of professional standards of practice in job situations
- Demonstrates comprehensive knowledge of case management principles and applicable standards
- Proficient in collecting and retrieving material from medical records
