About this role

3pm-11:30pm Monday Through Friday

We are seeking a highly skilled RN Liaison/Case Manager to join our growing team of healthcare professionals at Luxe at Wellington Rehabilitation Center. As a RN Liaison/Case Manager, you will supervise the clinical care planning and delivery processes, assess residents/clients upon admission and periodically as conditions change, and perform rounds to evaluate patient condition. Responsibilities

Supervise the clinical care planning and delivery processes to ensure the coordination of quality patient care. Reviews and updates the care plan accordingly.

Assess residents/clients upon admission and periodically as conditions change.

Perform rounds to evaluate patient condition.

Provide or assist with the delivery of direct patient care when indicated, including activities of daily living, meal assistance, medication and treatment administration, and leisure activities when indicated.

Qualifications

RN License

Minimum 5+ years nursing experience in a hospital, nursing home or other healthcare setting and a minimum 1-year experience as a nurse supervisor

CPR/BLS certified

Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care centers.

The ability to read, write, speak and understand the English language required, knowledge of other language helpful.
