Schedule:
PRN (as needed). As an ECT (Extended Care Team) Nurse at Transitions Hospice, you're the reinforcement — the skilled, composed clinical presence that steps in to ensure no patient need goes unmet. Responsibilities include delivering professional nursing care, conducting patient and family assessments, administering medications and treatments, identifying nursing diagnoses, recording clinical notes, communicating changes in patient condition to physicians, coordinating with community health partners, and maintaining clinical skills through continuing education.

Requirements
Active RN license in the state of employment
Active RN license in the state of employment
Graduate of an accredited school of Nursing
Clinical experience in hospice, home health, or a related setting preferred
Flexibility and confidence working across varied patient assignments
Strong assessment skills and sound clinical judgment under pressure
Excellent documentation habits and communication skills
Valid driver's license and willingness to travel locally
In-home healthcare experience
