*Overview* Join our new and upcoming dynamic healthcare team as a Home Health Nurse *(LPN/RN) *to providecomprehensive care in their home environment. As a vital member of our home health team, you will deliver personalized nursing services, promote health education, and ensure safe, effective treatment plans tailored to each individual's needs. - Conduct thorough initial patient assessments, including vital signs, physical examinations, and health histories to develop individualized care plans.
- Administer medications via injections, IV infusions, and other routes following strict aseptic techniques and medication protocols.
- Manage complex wound care procedures such as suturing, ostomy care, and dressing changes with precision and attention to infection control standards.
- Provide specialized treatments including botulinum toxin injections for therapeutic or cosmetic purposes and perform advanced procedures like catheterization or tube feeding management.
- Educate patients and their families on disease processes, medication administration, lifestyle modifications, and self-care techniques to promote independence.
- Coordinate with interdisciplinary teams using EMR (Electronic Medical Records) systems to document.
- Triage patient concerns via phone or in-person visits, prioritizing urgent needs such as acute pain management or infectious disease symptoms while adhering to HIPAA regulations.
*Requirements*
- Valid nursing license with current registration in Florida; *home health experience preferred.*
- Demonstrated proficiency with EMR/EHR systems for documentation and case management. *Experience using AXXESS software preferred BUT not mandatory*
- Experience working with diverse populations including seniors in assisted living facilities, residents in nursing homes or hospices, individuals with developmental disabilities or dementia care needs is a plus
- Strong clinical skills across multiple specialties including critical care (ICU experience), emergency medicine (ER experience), cardiology (cardiac catheterization), infectious disease care, and long-term chronic disease management.
- Knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding for procedures, and familiarity with laboratory specimen collection/processing.
- Ability to perform complex procedures such as IV insertions, phlebotomy and sterile technique practices confidently.
- Excellent communication and documentation skills for patient education, family teaching, and interdisciplinary collaboration
- Ability to work independently while maintaining high standards of safety and quality care.
