Job Responsibilities

As a Registered Nurse (RN) at Swan Lake Nursing & Rehabilitation, you will be responsible for:

Providing and/or supervising direct patient care, including assessing, planning, implementing, and evaluating care based on age-specific components

Coordinating care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction

Administering medications and procedures per established policies and guidelines

Collaborating with members of the interdisciplinary team to influence care outcomes

Requirements

Bachelor's degree in Nursing (BACHELORS)

Minimum 1 year of experience in a similar role (MINIMUM YEARS OF EXPERIENCE)

Current New York State Registered Nurse (RN) licensure
