Registered Nurse - Outpatient Hospice Case Manager

The Outpatient Hospice Case Manager registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community based upon community standards of practice. The nurse is responsible to improve or maintain patient's health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting. The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery. This position will support the West Team (ex: Lake Minnetonka area). Work Schedule: 4 days a week 8a-4:30p and every 4th weekend.
