Conducting patient assessments, developing individualized care plans, and implementing interventions as needed

Collaborating with other healthcare professionals to ensure seamless care transitions

Maintaining accurate and detailed records of patient care and treatment outcomes

Participating in quality improvement initiatives to enhance patient outcomes and satisfaction

Responsibilities

Provide direct patient care, including administering medication, taking vital signs, and performing treatments

Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams

Identify and address potential safety concerns and risks, reporting any incidents to supervisor

Maintain accurate and detailed records of patient care, treatment outcomes, and medication administration

Participate in patient education and support, promoting health literacy and self-advocacy
