We are seeking a dedicated and compassionate Registered Nurse to plan, organize, and direct patient care with an emphasis on community health education and hospice expertise. As a hospice RN, you'll draw on community resources to plan and deliver services that meet the needs of individuals and families in their homes and communities. Your nursing expertise will help ensure optimal comfort and quality of life by delivering comprehensive nursing care, collaborating with a multidisciplinary hospice team, and supporting patients and families through the end-of-life journey.

_Patient Care & Assessment_

  • Completes initial, comprehensive, and ongoing assessments of patients and families to determine needs, including a complete physical assessment and history of current and previous illnesses.
  • Provides professional nursing care utilizing all elements of the nursing process as defined in the state Nurse Practice Act.
  • Writes and initiates the plan of care, regularly re-evaluates patient and family/caregiver needs, and revises the plan of care as necessary.
  • Uses health assessment data to determine nursing diagnoses and develops a care plan with goals based on those diagnoses, incorporating palliative nursing actions and including the patient and family in the planning process.
  • Initiates appropriate preventive and comfort-focused nursing procedures; administers medications and treatments as prescribed by the physician.
  • Counsels patients and families in meeting nursing and related needs; provides health care instructions as appropriate.
  • Assists patients with activities of daily living, facilitating comfort and dignity throughout the end-of-life journey.
  • May act as Case Manager, coordinating patient care for an assigned caseload.

_Communication_

  • Completes, maintains, and submits accurate, timely clinical notes regarding patient condition and care given, including pain/symptom management changes and outcomes.
  • Communicates with physicians regarding patient needs, reports changes in condition, and obtains/receives orders as required.
  • Coordinates the care plan with community health-related resources and personnel.
  • Teaches patients and family/caregivers self-care techniques, medication management, diet, and other instructions as ordered.
  • Maintains a safe environment for the patient.
  • Works closely with family/caregivers and the hospice interdisciplinary team to support the emotional and physical needs of patients and families.
  • Attends and actively participates in interdisciplinary group meetings.

*Requirements*

  • Graduate of an accredited school of nursing.
  • One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent home care experience.
  • Current RN licensure in-state and current CPR certification.
  • Bachelor's degree with one (1) year of hospice care experience preferred, not required.
  • Licensed driver with an automobile insured in accordance with state and/or organization requirements and in good working order.
  • Ability to supervise hospice aides (management experience not required).
  • Excellent observation, verbal and written communication, problem-solving, and basic math skills.

Pay: $34.00 - $42.00 per hour
