Gulfside Health & Rehab is seeking an experienced and detail-oriented Registered Nurse (RN) MDS Coordinator to join our team in Clearwater, FL. What You’ll Do

Complete and oversee MDS assessments and care planning in accordance with federal and state regulations

Ensure accuracy and timeliness of MDS submissions

Collaborate with interdisciplinary teams to optimize reimbursement and quality outcomes

Monitor case mix index (CMI) and identify opportunities for improvement

Maintain compliance with Medicare/Medicaid guidelines and facility policies

Participate in audits and support survey readiness

What We’re Looking For

Active RN license in the state of Florida (required)

1+ years of MDS experience in a skilled nursing facility (required)

Strong knowledge of RAI process, PDPM, and Medicare guidelines

Detail-oriented with strong organizational and communication skills

Ability to work independently and as part of a collaborative team
