Home Health Professionals Network (HHPN) is seeking compassionate Registered Nurse Case Managers to provide skilled home health services throughout Hamden and surrounding communities.
In this role, you will combine clinical knowledge with relationship-based care. You will assess patients, coordinate services, provide education, monitor progress, and partner with families and providers to support the best possible outcomes.
Key
Responsibilities
- Complete skilled nursing visits and patient assessments
- Manage assigned patients and coordinate care services
- Follow physician orders and individualized plans of care
- Administer medications and treatments
- Monitor patient progress and communicate changes
- Educate patients and caregivers
- Maintain accurate and timely documentation
- Collaborate with the HHPN clinical and administrative teams
Qualifications
- Active Connecticut Registered Nurse license
- Home health or homecare experience preferred
- Current BLS certification
- Excellent assessment and communication skills
- Reliable transportation
- Strong organization and time-management abilities
- Commitment to compassionate, high-quality care
