What You’ll Do

• Provide skilled nursing care to patients in their homes

• Conduct comprehensive assessments and ongoing evaluations

• Develop and revise individualized care plans in collaboration with the care team

• Educate patients and families on disease process, medications, and care strategies

• Administer treatments and medications in accordance with physician orders

• Serve as Case Manager for your assigned patients, coordinating services and resources

• Communicate effectively with the interdisciplinary team to ensure continuity of care

Schedule Expectations

• Full-time | 5 days per week

• Shared on-call rotation

What You’ll Need

• Active RN license in the State of New Hampshire

• Graduate of an accredited nursing program; BSN preferred

• 1–2 years of recent acute care or home health experience

• CPR certification

• Valid driver’s license and reliable, insured vehicle

• Strong clinical skills, excellent communication, and a heart for community-based care
