We are seeking a compassionate, reliable, and experienced *Registered Nurse (RN)* to join our growing Home Health team. The Home Health RN will provide skilled nursing services to patients in their homes, helping them achieve optimal health outcomes while maintaining comfort, dignity, and independence.
Responsibilities
* Perform comprehensive nursing assessments and develop individualized care plans.
* Provide skilled nursing care in accordance with physician orders and the patient's plan of care.
* Monitor patient condition and identify changes in health status.
* Administer medications, treatments, injections, and other skilled nursing services as ordered.
* Educate patients and caregivers regarding medications, disease management, treatments, and safety.
* Perform wound care and other appropriate skilled nursing procedures.
* Coordinate care with physicians, therapists, social workers, caregivers, and other members of the interdisciplinary team.
* Complete accurate and timely clinical documentation in the agency's electronic medical record system.
* Communicate significant changes in patient condition to the physician and appropriate members of the care team.
* Participate in case conferences and care coordination as needed.
* Follow all applicable Medicare, state, federal, and agency policies and regulations.
Qualifications
* Current and unrestricted *California Registered Nurse (RN) license*.
* Current CPR/BLS certification.
* Previous home health experience preferred.
* Strong clinical assessment and critical-thinking skills.
* Excellent communication and interpersonal skills.
* Ability to work independently and manage a patient caseload.
* Reliable transportation and valid driver's license.
* Ability to complete required documentation accurately and within agency timelines.
