If you are passionate about making a meaningful difference in the lives of patients and their families, we would love to hear from you. *Position Overview* The *Hospice Field Admission Nurse (RN)* serves as the first point of contact for patients entering hospice care, playing a critical role in guiding patients and families through the admission process with empathy and expertise. In this field-based role, you will: * Conduct initial physical and psychosocial assessments * Determine hospice eligibility based on Medicare guidelines * Develop individualized plans of care and goals * Provide education on hospice philosophy and services * Support patients, families, and caregivers as they begin their hospice journey Assessments may take place in private homes, hospitals, or skilled nursing facilities. *Key Responsibilities* * Admit eligible patients to hospice services in accordance with Medicare Conditions of Participation * Educate patients and families on hospice care, services, and expectations * Guide patients, caregivers, and community partners through the admissions process * Collaborate with Clinical Leadership, physicians, Intake Coordinators, and Liaisons * Utilize strong clinical judgment to evaluate hospice eligibility per Medicare guidelines * Ensure *accurate, timely, and compliant documentation* in the electronic medical record (*HospiceMD*) to support regulatory standards, audit readiness, and quality patient care *Qualifications* * Active California RN license in good standing * Minimum of 1 year of nursing experience * Strong clinical assessment and communication skills * Ability to learn and navigate electronic medical record systems (experience with *HospiceMD* is a plus) * Valid driver’s license and reliable transportation * Willingness to travel within a 60-mile radius * Availability to work weekends *Education* * Associate Degree in Nursing (required) * Bachelor of Science in Nursing (BSN) preferred *Benefits* * Paid training * Competitive compensation * Supportive, team-oriented environment * Opportunities for growth and development * Medical, dental, and vision insurance * 401(k) plan * Paid time off and holiday pay * Mileage reimbursement *Additional Information* *Schedule Requirements: * * Must be available from 11:30am – 8pm or 12pm - 8:30pm * Weekend availability required *Location:* * San Diego, CA (required) * Field-based role *Job Type:* Full-time Job Type: Full-time Pay: $53.00 - $58.00 per hour Benefits: * 401(k) * Health insurance * Paid time off * Vision insurance Application Question(s): * Are you available 11:30am - 8pm and weekends?
