Summary
Polaris Healthcare and Rehabilitation Center, a leading provider of healthcare services in Milford, Delaware, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team! Responsibilities
Provide direct patient care under the supervision of a Registered Nurse (RN) or other licensed healthcare professionals
Administer medications, treatments, and procedures as prescribed by healthcare providers
Monitor and record vital signs, medical conditions, and treatment outcomes
Assist with invasive and non-invasive procedures, including IV therapy and wound care
Collaborate with other healthcare professionals to develop and implement individualized care plans
Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions

Requirements
Active Delaware LPN License: Possess an active LPN license in the state of Delaware.
