Licensed Practical Nurse (LPN)
The Cascades Health and Rehabilitation Center is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our growing team! The Licensed Practical Nurse (LPN) is responsible for providing direct patient care under the supervision of a registered nurse or physician. The LPN will be responsible for administering medications, taking vital signs, performing wound care and assisting patients with daily living activities. The LPN will also be responsible for documenting patient care in the electronic health record and liaising with other healthcare professionals as needed.
Licensed Practical Nurse (LPN)
Responsibilities
- Administer medications as prescribed by a physician or registered nurse
- Monitor and record patient vital signs, such as blood pressure and heart rate.
- Communicate with patients and their families about their care and treatment plan.
- Document patient visits and treatment plans in electronic medical records (EMR)
- Collaborate with other healthcare professionals, such as registered nurses, physicians, and therapists.
- Complete patient assessments, coordination of care
- Identify and reports changes in resident’s condition to Registered Nurse
- Follow infection control and safety protocols to ensure patient and staff safety.
- Maintain confidentiality and ethical standards in all patient interactions.
