RN Care Coordinator - Gynecology Oncology

Cleveland Clinic Hillcrest Hospital is seeking an RN Care Coordinator for the Department of Hematology & Medical Oncology. The role involves working collaboratively with a multidisciplinary care team across the continuum of care for high-risk patients, conducting targeted outreach, providing care and disease management coordination, conducting comprehensive clinical assessments, serving as a liaison and advocate for patients and families, assisting in managing transitions of care, and working on quality and process improvement initiatives. The position works days from 7:00 a.m. to 5:30 p.m.
