Medfamily Wellcare Center is currently seeking experienced, compassionate, dependable, and highly motivated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our Home Health team in Miami-Dade County.
Qualified candidates must have previous Home Health experience, strong clinical assessment skills, excellent communication abilities, and the ability to work independently while delivering high-quality patient care.
Responsibilities
- Perform skilled nursing visits in patients' homes.
- Complete nursing assessments and provide skilled interventions within the scope of licensure.
- Monitor patient conditions and promptly report significant changes.
- Provide wound care, medication management, and patient/caregiver education.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, and the Director of Nursing.
- Follow physician orders, individualized plans of care, and agency policies.
- RNs perform SOC, ROC, Recertification, and Discharge assessments, including OASIS documentation, as assigned.
- LPNs provide skilled nursing care under required RN supervision and direction.
- Maintain compliance with applicable Medicare, AHCA, and agency requirements.
