Responsibilities
- Lead discharge planning and care coordination for acute hospital patients across the life span, from neonatal to geriatrics
- Conduct needs assessments, identify post-discharge needs, and coordinate DME ordering and community services
- Develop and document individualized plans of care and monitor progress; ensure alignment with CMS processes
- Collaborate with Social Services and the interdisciplinary team to arrange placements and services after discharge
- Manage a patient load in the approximate range of 25–30 patients per day and document in the EPIC charting system
Requirements
- Registered Nurse (RN) license; pending license accepted
- BLS certification
- Minimum 1 year acute care experience with discharge planning / care coordination in an acute hospital
- Experience in discharge planning and care coordination; diverse case management across Med Surg, Long LOS (LLOS), and ED preferred
- EPIC charting experience; ability to float to all hospital areas; travel assignment in Everett, WA; locals not accepted
