About this role

The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Completes initial, comprehensive and ongoing assessment of patient and family to determine hospice needs. Provides professional nursing care by utilizing all elements of nursing process. Develops care plan with palliative nursing actions. Acts as Case Manager when assigned. Participates in on-call duties. Supervises ancillary personnel.

Requirements

  • RN License
  • Current licensure in State
  • RN License
  • Graduate of National League of Nursing accredited school of nursing
  • Current licensure in State
  • Minimum of 1 year experience as a professional nurse within the last 3 years OR baccalaureate degree in nursing from NLN accredited program and current RN license
  • Minimum of 2 years experience preferred, at least one in public health, home care, or hospice nursing
  • Bachelor's degree in nursing
  • Hospice experience
  • In-home healthcare experience
