About this role
*Service Area:* _Patients will be assigned within a 40-mile radius from where the nurse resides_
*Job Type:* PRN
*Job Description:* We are seeking a dedicated and compassionate *Registered Nurse (RN)* to join our home health team. This is a *PRN*, offering flexibility and rewarding one-on-one patient care. While prior home health experience is preferred, we are open to training motivated nurses who are new to this setting.

Responsibilities

- Deliver skilled nursing care to patients in their homes according to physician orders.
- Supervise and collaborate with LVNs, and other care staff.
- Conduct and complete all required OASIS documentation, including admissions, recertifications, resumptions of care, and discharges.
- Exercise sound clinical judgment and independent decision-making.
- Accurately complete electronic documentation.

Qualifications

- Active RN license in the state of Texas.
- Diploma, Associate, or Bachelor’s degree in Nursing.
- Minimum of one (1) year of home health experience preferred, but not required and/or
- Minimum of one (1) year of acute short-term hospital experience preferred, but not required.
- Current CPR certification.
- Strong critical thinking and problem-solving abilities.
- Excellent communication and interpersonal skills.
- Self-motivated and able to work independently.
- Valid driver's license and proof of auto insurance.
- Comfortable using electronic health records (EHR) systems.
