Job Summary
We are a newly licensed, 3-category Private Home Care Provider (PHCP) in Georgia authorized to deliver Companion Sitter, Personal Care, and Skilled Nursing Services. We are seeking a compliant, detail-oriented Registered Nurse (RN) based in or near Forsyth County to serve as our Fractional Director of Nursing / Nurse Supervisor on Record.
This is a low-overhead, compliance-focused startup role perfect for an experienced RN, nurse consultant, or retiree residing in the North Metro Atlanta area who wants a highly flexible side position. You will ensure our agency maintains strict adherence to Georgia Department of Community Health (DCH) regulations while we prepare to launch operations and onboard our first private-pay clients in the Cumming area.
Core Responsibilities (Startup Phase)
- Compliance Oversight: Serve as our licensed RN Supervisor on record with the Georgia DCH and manage our compliance profile in the GAINs portal.
- Policy Customization: Review and sign off on our initial clinical policies, intake forms, and emergency preparedness plans to ensure local and state compliance.
- Audit Readiness: Conduct brief monthly audits of blank employee and client file templates to ensure total readiness for the unannounced initial DCH state survey.
- Scalable Clinical Care: Conduct initial in-home client assessments throughout Forsyth County, build custom plans of care, and perform required 92-day supervisory visits (paid separately per service) as clients onboard.
Qualifications & Requirements
- Active License: Unencumbered Registered Nurse (RN) license in the State of Georgia.

Experience
Minimum of 2–3 years of experience in Home Health, Private Home Care, or a clinical leadership role preferred.
- DCH Familiarity: Strong working knowledge of Georgia DCH Rule 111-8-65 regarding private home care provider regulations.
- Background Check: Must be willing to clear the mandatory Georgia Criminal History Record Restrictions (GCIC) fingerprint background check immediately upon hire.
- Location: Must reside in or within a 30-minute driving radius of Cumming / Forsyth County to comfortably manage local client assessments.
Compensation Structure
- Monthly Base Retainer: $100 – $150/month to maintain the license on record and complete minimal administrative compliance tasks while we have zero clients.
- Per-Assessment Add-Ons: Paid a flat rate of $150 per initial client assessment and plan of care setup.
- Growth Path: Hourly rate and weekly hours will automatically scale up under a tiered contract structure as the agency transitions from 0 to active clients.
