Hospice Nurse - LPN

*Hospice Nurse - LPN Key Responsibilities*

• Deliver skilled nursing care in alignment with the patient’s plan of care.
• Assist in assessing physical, psychosocial, and environmental patient needs.
• Participate in developing and implementing patient care plans.
• Ensure continuity and quality of care through timely documentation and teamwork.
• Communicate effectively with patients, families, team members, and referral sources.
• Participate in interdisciplinary meetings, in‑service training, and quality improvement programs.
• Follow infection control and safety procedures.
• Uphold agency policies and professional standards.

*Hospice Nurse - LPN Minimum Requirements*

• Graduate of an accredited LPN program.
• Current LPN state license
• Current CPR certification
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong clinical and interpersonal skills.
• Minimum 2 years of clinical nursing experience; hospice experience preferred.

*Work Environment*

• Care provided in patient homes under varying conditions.
• Exposure to infectious diseases, blood, and bodily fluids possible.
• Requires local travel and ability to work a flexible schedule.
• May involve prolonged standing, walking, and lifting up to 50 lbs.

Job Type: Full-time Pay: $28.00 - $35.00 per hour
