This opportunity is ideal for an RN who is comfortable working independently in patients' homes, completing comprehensive clinical assessments, coordinating care, and maintaining accurate and timely documentation. Responsibilities Complete Start of Care assessments and initial patient evaluations Perform skilled nursing visits, reassessments, resumptions of care, recertifications, and discharge visits Complete comprehensive nursing assessments and identify changes in patient condition Develop, review, and update individualized plans of care
