About Matrix Home Care
Matrix Home Care is seeking an experienced, compassionate, and dependable Registered Nurse to complete Start of Care assessments and skilled nursing visits throughout Kissimmee and the surrounding Osceola County area.
This opportunity is ideal for an RN who is comfortable working independently in patients' homes, completing comprehensive clinical assessments, coordinating care, and maintaining accurate and timely documentation.
Responsibilities
Complete Start of Care assessments and initial patient evaluations
Perform skilled nursing visits, reassessments, resumptions of care, recertifications, and discharge visits
Complete comprehensive nursing assessments and identify changes in patient condition
Develop, review, and update individualized plans of care
Provide wound care, medication management, disease-management education, and other skilled nursing services
Educate patients and caregivers regarding medications, treatments, safety, and care instructions
Coordinate care with physicians, therapists, caregivers, and the clinical team
Complete accurate and timely OASIS and electronic medical record documentation
Report significant findings and changes in condition promptly
Follow agency policies, professional nursing standards, and regulatory requirements
