Licensed Vocational Nurse - Field Nurse

• Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to develop personalized care plans.
• Administer medications via injections, IV infusions, catheterizations, and other routes in accordance with physician orders and safety protocols.
• Provide specialized care for individuals with complex needs such as Alzheimer’s disease, dementia, or memory impairments, ensuring comfort and safety.
• Manage wound care, including dressing changes and infection control measures, utilizing aseptic techniques.
• Collaborate with interdisciplinary teams to coordinate case management, discharge planning, and ongoing treatment strategies.
• Document all patient interactions accurately using Electronic Health Record (EHR) systems like Epic or Cerner while maintaining strict compliance with HIPAA and confidentiality policies.
• Educate patients and their families on health management strategies, medication administration, nutrition, and safety measures to promote optimal health outcomes.

*Skills*

• Valid state licensure as a Licensed Vocational Nurse with current certification to practice independently in the field.
• Extensive experience working with diverse populations with pediatric patients, individuals with developmental disabilities or chronic conditions such as acute pain or infectious diseases.
• Proficiency in using EMR/EHR systems such as Axxess for documentation and clinical workflows.
• Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and long-term care regulations.
• Skilled in performing clinical procedures including venipuncture, IV insertion/infusion, catheterization, tube feeding management, and airway management.
• Experience working in various healthcare settings such as hospitals (Level I Trauma Center or ICU experience preferred), skilled nursing facilities, hospice care environments, or home & community care.
• Ability to triage patient needs effectively through assessment of symptoms like acute pain or infectious disease symptoms; capable of providing first aid and emergency response when necessary.
• Excellent caregiver communication skills to work collaboratively with patients’ families and multidisciplinary teams while maintaining clinical confidentiality policies.
