Evergreen Healthcare is hiring compassionate Registered Nurses (RN) to support and supervise Personal Care services delivered to clients in their homes. Bilingual candidates are strongly encouraged to apply.

*Qualifications of the Registered Nurse (RN):*

Read the full posting
  • Valid and active RN license in the state of practice.
  • Minimum of 6 months of hands-on nursing experience.
  • Experience with personal care or home-based services (preferred).
  • Current CPR certification.
  • Must be willing to travel locally for in-home client visits.
  • Bilingual preferred.

*Responsibilities of the Personal Care RN:*

  • Conduct initial and ongoing assessments of clients receiving personal care services.
  • Develop individualized care plans in coordination with physicians and family members.
  • Supervise and provide clinical guidance to Personal Care Aides (PCAs).
  • Perform periodic reassessments in compliance with Medicaid/insurance regulations.
  • Maintain accurate and timely documentation in the system of record.
  • Promote health, safety, and quality of care in the home setting.
