Position Overview
- Role: Registered Nurse (RN) – Case Manager (Level 2, CHHA)
- Location: Rochester, New York
- Assignment Length: 13 Weeks
Schedule
Monday–Friday | 8:00 AM – 4:30 PM
Shift: 5x8s (Day Shift) Weekend Requirement: As needed
Minimum Requirements
Active New York RN License
BLS Certification (any provider accepted)
Certified Home Health Care experience
Strong knowledge of OASIS documentation
Telemetry experience
Experience with EPIC or Netsmart
Valid NYS Driver’s License
Minimum
1 year hospital RN experience
Home care experience strongly preferred
