Schedule
Weekday, One weekend day per month
- Hours: 8-
- 12 hour shift
- Setting: One-on-one care in the client’s home
Responsibilities
- Seizure monitoring and observation
- Medication administration
- Feeding assistance (per care plan)
- Documentation of care provided
- Communicate any changes in condition to the clinical team
- Provide *skilled nursing care* for a medically fragile infant in accordance with the plan of care.
- Monitor vital signs and assess for any changes in the child’s condition, intervening as necessary.
- Assist with daily activities, including repositioning, range of motion exercises, and developmental support.
- Maintain accurate documentation of all assessments, treatments, and interventions.
- Educate and support family members on care techniques and emergency procedures.
- Collect information about conditions and treatment plans from caregivers, nurses, doctors and family members
- Accurately monitor and track clients’ overall health and medical history during every visit
- Take and record measurements of blood pressure, temperature, heart rate etc.
- Verify clients are taking the correct dosages of medications
- Help clients complete physical therapy and other recommended exercises
- Remain on-call for clients who have healthcare emergencies
- Provide emotional and psychological support to the client and family members as needed

- *Pediatric experience preferred* *-* experience with *tube and suction care required*.
- Strong *clinical assessment and critical thinking skills*.
- CPR certification
- Compassionate, patient, and dedicated to providing high-quality care.
- Ability to work independently while coordinating with the child’s healthcare team.
